Cardiac Troponin I/T (cTnI, cTnT)
Blood proteins released when heart muscle is injured; the primary laboratory marker in the evaluation of possible acute myocardial infarction.
What it is for
Supports the detection of myocardial injury in patients with suspected acute coronary syndrome, interpreted together with ECG findings and clinical presentation using a serial (0/1-hour or 0/2-hour) testing strategy. High-sensitivity assays detect lower concentrations and allow earlier rule-out pathways in emergency departments.
Limitations
Elevated troponin confirms myocardial injury but not its cause — non-ACS etiologies (sepsis, renal failure, pulmonary embolism, myocarditis) also elevate values. Assays are not interchangeable: each platform has its own decision thresholds and reference-change values. Serial testing is required; a single value cannot rule in or rule out infarction.
Before you test
No fasting is required. Tell clinical staff about recent strenuous exercise, kidney disease, or prior heart procedures, all of which can affect interpretation.
